To protect your own privacy and the privacy of others, please do not include any personal information including phone numbers and email addresses in your pledge. Please do not share personal information about others.

Health Quality Ontario will be collecting, using and disclosing your personal information with your consent to collect information to facilitate change and improvement by providing a forum for people to pledge to commit to a change, improvement and/or innovation. Participation in Change Day Ontario is voluntary.
By posting a pledge on changedayontario.ca website, you are giving your consent for Health Quality Ontario to collect, use and disclose your personal information such as photos or videos or opinions that you may post on this website.
By participating in changedayontario.ca you agree to only disclose personal information about yourself and not about any other person.
This consent becomes effective when you click on the agree button and expires three years from that date.

Health Quality Ontario’s collection of personal information is governed by the Freedom of Information and Protection of Privacy Act.
Health Quality Ontario will be using your personal information submitted by you by virtue of your participation in the initiative to identify you and your contributions to the changedayon.ca site. We may promote your contribution through social media as part of the Change Day Ontario campaign. Occasionally we may use your email address to contact you so we can highlight your pledge and share it across a broader audience through a newsletter or other format. We may also use your email to introduce you to new features and functionality of our website and social media platforms, provide you with service information and allow you to send out invitations to join the Change Day Ontario campaign.
Health Quality Ontario will be disclosing any photos, videos or opinions in your pledge to the changedayontario.ca website. If we choose to use information in your pledge for research or educational purposes, we will maintain strict anonymity and will not include any information that could identify you. We do not disclose personal information to third parties unless required by law.

Health Quality Ontario keeps information on its server in log files including information such as the time of access, browser type, operating system, etc. This information is used to ensure quality of service and to track any unauthorized use. We also use it to analyze trends in site traffic, and to improve the content and organization.
Health Quality Ontario will retain your personal information for 3 years after which it will be securely destroyed, unless you consent to its earlier disposal. Health Quality Ontario will ensure your personal information is readily available, should you request to review it.
